چکیده

THE RATE OF SEVERE SEPSIS HOSpitalizations has doubled during the last decade with estimates indicating that at least 750 000 persons are affected annually in the United States. Approximately, 500 000 patients with severe sepsis in the United States annually are initially treated in emergency departments. The Surviving Sepsis Campaign international consensus guidelines recommend protocol-driven treatment that uses quantitative resuscitation for emergency department patients with severe sepsis and septic shock. Quantitative resuscitation refers to the use of an explicit protocol that targets predefined physiological or laboratory goals to be achieved within the first several hours. This concept was pioneered by Shoemaker et al to treat high-risk surgical patients. Results of a recent meta-analysis indicated a survival benefit associated with the use of

Lactate Levels Can Guide Early Resuscitation Goals in Severe Sepsis Lactate Clearance vs Central Venous Oxygen Saturation as Goals of Early Sepsis Therapy: A Randomized Clinical Trial

Background: The Surviving Sepsis Campaign international consensus guidelines recommend early resuscitation to optimize oxygen delivery. Although the best measure of oxygen delivery is debated, the published protocol utilized central venous oxygen saturation (ScvO2) of >70%. Serum lactate has been shown to prognosticate outcome, with higher levels being associated with worse morbidity and mortal...

Lactate clearance vs central venous oxygen saturation as goals of early sepsis therapy: a randomized clinical trial.

CONTEXT Goal-directed resuscitation for severe sepsis and septic shock has been reported to reduce mortality when applied in the emergency department. OBJECTIVE To test the hypothesis of noninferiority between lactate clearance and central venous oxygen saturation (ScvO2) as goals of early sepsis resuscitation. DESIGN, SETTING, AND PATIENTS Multicenter randomized, noninferiority trial invol...

Lactate revisited : is lactate monitoring beneficial for ICU patients ? REVIEw

Blood lactate levels are frequently measured in critically ill patients. Whilst these measurements are known to be accurate, for the correct interpretation of hyperlactataemia, it is essential to have sufficient understanding of anaerobic and aerobic mechanisms of production and clearance. The consistency of the prognostic value emphasizes the place of lactate measurement in the risk-stratifica...

The value of venous oximetry.

PURPOSE OF REVIEW Maintenance of adequate tissue oxygenation is an important task in intensive care units. In this context, venous oximetry by obtaining mixed venous oxygen saturation or central venous oxygen saturation has been discussed as useful monitoring parameters. This review discusses the physiology and clinical application of these parameters. RECENT FINDINGS No study has so far demo...
